A Cardiac Resynchronization Therapy Defibrillator (CRT-D) is an advanced implantable device that combines two life-saving functions in one: it resynchronises the contraction of the heart's two ventricles to help a weak heart pump more efficiently, and it acts as a defibrillator to prevent sudden cardiac death from dangerous fast heart rhythms. It is specially designed for patients with heart failure whose heart chambers are no longer beating together in a coordinated way.
In a healthy heart, the left and right ventricles contract together, pumping blood efficiently to the body. In many patients with heart failure, this electrical timing becomes disrupted — the ventricles contract out of sync (a condition often seen with a wide QRS / left bundle branch block on ECG). This makes the heart's pumping action weaker and worsens heart failure symptoms.

The procedure is performed under local anaesthesia with mild sedation, similar to a pacemaker implant. A small incision is made below the collarbone, and the three leads are carefully guided into position under X-ray guidance. The device is then connected and placed in a pocket under the skin. The procedure typically takes 1–2 hours, with most patients going home within 1–2 days.
CRT-P (Cardiac Resynchronization Therapy Pacemaker) provides resynchronisation only, without defibrillator backup.
CRT-D combines resynchronisation with defibrillator protection, making it suitable for patients who need both benefits.
AICD provides defibrillator protection alone, without resynchronisation pacing.
